Case Manager
Brief Job Description
We are seeking experienced candidates to perform Case Management for Behavioral Health Services. Case managers will work with clients of various age groups and with various DSM-5 diagnoses. We would like candidates who are dedicated to the mental health /behavioral health field, and have a desire for continued growth, learning, and practice in the profession. Case Managers will perform various clerical and clinical tasks for clients associated to their caseloads. Case managers will work along with the clinical team to meet client’s needs as well as organizational needs. Selected candidates will conduct case management duties with clients primarily in the client’s home or other places within the community. Some work may be online however all clinical services must be delivered in-person. Progress notes for the services rendered are required to be submitted on a weekly basis.

Qualifications
· Applicants must have a minimum of a Bachelor’s degree in the following fields: Counseling, Social Work, Sociology, or Psychology

Candidates will provide community-based services which will include psychosocial rehabilitation to clients within in the community. Work is primarily conducted in the client’s home, or other places within the community. In-person services are required for psychosocial rehabilitation which may be delivered by a non-licensed staff at a bachelors or masters level. Progress notes for the services rendered are required to be submitted on a weekly basis.

We would like candidates who are dedicated to the mental health /behavioral health field, and have a desire for continued growth, learning, and practice in the profession. Candidates will provide community-based services which will include supportive counseling services, as well as psychosocial rehabilitation to clients within in the community. Work is primarily conducted in the client’s home, or other places within the community. Some remote services are allowed for supportive counseling which will only be delivered by Licensed Clinicians. Progress notes for the services rendered are required to be submitted on a weekly basis.

Qualifications
· Applicants must have a minimum of a Master’s degree in the following fields: Counseling, Social Work, Sociology, or Psychology and must have a Practicing license from their board of practice (ex. PLPC, LPC, LCSW)

Breif Job Description
We are seeking an experienced candidate to conduct Assessments for Behavioral Health Services for various DSM-5 diagnoses. We would like candidates who are dedicated to the mental health /behavioral health field, and have a desire for continued growth, learning, and practice in the profession. The selected candidate will conduct Assessments with clients primarily in the office, and whenever necessary in the client’s home or other places within the community. Some services may be delivered via telehealth. This position is preferably an in-office position.

We are seeking an experienced candidate to provide Clinical Management for Behavioral Health Services for various DSM-5 diagnoses. We would like candidates who are dedicated to the mental health /behavioral health field, and have a desire for continued growth, learning, and practice in the profession. The selected candidate will be expected to provide clinical guidance and direction to clinicians to ensure sound clinical treatment is provided.
Candidates are expected to meet with clinicians regularly for supervision and training, review documentation for appropriateness. document findings and provide feedback to clinicians, conduct peer reviews with MCOs, as well as meet with clients as needed. This position is an in-office position. Work is primarily in the office, and whenever necessary in the client’s home or other places within the community. Some work may be delivered online or via telehealth.
